Animal Type/Breed

* Status (lost or found)	Lost Found
* Contact Name	A value is required.
* Contact Phone	A value is required.
* Sex	Male Female
* Description	A value is required.
Registration Number
* Street (where animal was lost or found)	A value is required.
* Town (where animal was lost or found)	A value is required.
* Date Lost/Found	Invalid format.A value is required.
Photo	(Please only submit JPG files that are no more than 500KB.)
